BuShips Directive 1111-01 was issued 11 November 2011 under the authority of the Third Space Lord. It commissions HMS Chaos, the second ship in Third Fleet.

My fellow Manticorans,
Effective as of 0001 Hours on November 12, 2011, it is my pleasure to announce the commissioning of the HMS Chaos, DD-57. This is a Havoc Class Destroyer under the Command of Captain (j.g.) Daniel Walker and is assigned to the Third Fleet.
The Executive Officer of this Ship is Commander Stephanie Taylor
The CPO for this Ship is Chief Petty Officer Brandi Warren
As of the date of Commissioning, the following additional personnel are assigned to the HMS Chaos:
- Spacer Third Class Carl Christianson
- Spacer Third Class Joe Matthews
- Spacer Third Class Jack Warren
