BuShips Directive 1204-03 was issued 23 April 2012 under the authority of the Third Space Lord. It commissions HMS Galahad.

My fellow Manticorans,
Effective as of 0001 Hours on April 23, 2012, it is my pleasure to announce the commissioning of HMS Galahad, DD-478. This is a Roland Class Destroyer under the Command of Lieutenant Commander Timothy Bailey and is assigned to the Second Fleet.
The crew of this ship are as follows:
- LCDR Timothy Bailey (and assigned as CO of HMS Galahad)
- LT (s.g.) David Poll (and assigned as XO of HMS Galahad)
- CPO Rochelle Weber (and assigned as Bosun of HMS Galahad)
- Commodore Penny Horwitz
- Spacer 3rd Patrick Bailey
- Spacer 3rd Don Eggert
- Spacer 3rd Scott McAlister
The following persons are breveted in rank:
- Timothy Bailey to Lieutenant Commander (and assigned as CO of HMS Galahad)
- David Poll to Lieutenant, senior grade (and assigned as XO of HMS Galahad)

Addendum issued 15 May 2012
Ladies and gentlemen, Due to an oversight on my part, there are three memebrs who were not properly breveted in rank as requested when HMS Galahad commissioned. Therefore, I am rectifying that error and ommission. Effective immediately, the following crewmembers of HMS Galahad are breveted to the ranks indicated:
- Patrick Bailey, Sergeant of Marine contingent, brevetted E-5
- Don Eggert, Helmsman First Class, brevetted E-3
- Scott McAllister, Spacer Second Class, brevetted E-2
