''Siren'' was first brought into commission on 28 November 2016 under the command of [[CDRE]] Eddy Roberts by order of [[Bureau of Ships|BuShips]]. The crew combined the crews of {{hms|Jonas Adcock|CA-489}} and {{hms|Helen Zilwicki|CA-548}}. When the ''Adcock's'' command staff was relieved, [[CDRE]] Roberts decided to combine both ships into the ''Siren.''
